If an Aetna insurance card has "Indemnity" or "Traditional" on it (it will be clearly stated) it is NOT under HSM. If one of the two words is NOT on the insurance card, it is managed through HSM and will require the appropriate PA based on time/visits. See Judy Machal's card for example of HSM managed Aetna card.
