When new patients come and and you copy their insurance card, PLEASE check to make sure all the numbers are ledgible and are not cut off. Also, check to see if there is an ID number on the card- if not, please make sure to get the subscribers social security number. Ask the patient if they are the card holder or if it the spouse/parent. Whoever is the card holder, we need to get their ssn.
