Just a reminder that when there is a new patient or update to PLEASE send back either file (new pt) or copy of insurance card (update) with the fee slip. I need this info when I enter the patient/charges into the computer.

4th of July Special!

We are running the 4th of July Special and I have a copy of the ad up front. New or existing pt's are able to receive a consultation, exam, testing, and any necessary x-rays for only $17.76- to be paid at the time of service. This is a one visit deal. After that the pt will be required to either pay regular cash rates or go through insurance. If the pt wants the initial visit to also be submitted to insurance, they have the option to receive an hour massage instead for only $17.76. This special runs July 1-July 15.

Maternity Leave

I have decided to make July 19th my last day. If all goes well, I plan on picking up my Saturday hours again as soon as possible. Otherwise, I should be back in about six weeks. In the event that I go a little early, I have a massage scheduled on Tuesday, July 22nd that would need to be canceled. Thank You Everyone!!! ~April

health pass 2

For now, every new patient will be getting 1 business card and 2 health passes in their ROT folder. They will also be getting one of the health pass cards for themselves (ie each new pt receives a 1/2 hr massage).

Everyone is to hand out 3 health passes to 5 patients or other folks outside of the clinic during out health pass week. Please put a 30 day expiration date on each card you hand out as well as your initials. thanks, pam

Same Patient Name

We now have 2 patients with the name Karen M. Anderson. The newest one is differentiated in Medisoft with an (83) after Anderson. This is for the year she was born. The other one was born in 68. PLEASE make sure you are getting the correct one. If possible, verify birthdate and write it on the fee slip so I make sure I am entering the charges for the correct one-ohtherwise we will have a mess with insurance if the wrong one gets entered. Please see me with any questions.

Also, for any updates (longer than say, 6 months) please make sure you are verifying their address, phone and insurance so I can make sure Medisoft is up to date.

NEW PATIENT INSURANCE INFO

When new patients come and and you copy their insurance card, PLEASE check to make sure all the numbers are ledgible and are not cut off. Also, check to see if there is an ID number on the card- if not, please make sure to get the subscribers social security number. Ask the patient if they are the card holder or if it the spouse/parent. Whoever is the card holder, we need to get their ssn.

MASSAGE ONLY PATIENTS

Just a reminder that if someone is coming in only for massage, please do not set them up with a file in Medisoft. They are put under a generic CASH account, that way they are not taking up space in Medisoft, and it is easier to keep track of them. I will put them in Medisoft if/when they become a chiropractic patient.

Nutritionals

Whenever you sell any nutritionals, PLEASE make sure to break it down on the fee slip of how much is for the product and how much is for tax.

Per SK- Please make sure you write up a receipt for ALL cash transactions- whether or not the patient wants one. It is our only method of tracking cash payments.

FYI-
There are now new forms to use for:

Request for Information....from other clinics
Release of Information.......to other clinics

There are separate files for these in the drawer, and they can replace the old 1/2 sheet forms used in the past.

Aetna and HSM

If an Aetna insurance card has "Indemnity" or "Traditional" on it (it will be clearly stated) it is NOT under HSM. If one of the two words is NOT on the insurance card, it is managed through HSM and will require the appropriate PA based on time/visits. See Judy Machal's card for example of HSM managed Aetna card.

I will be able to come in on 4/2 and 4/3 am. Also, next week Wendy and I are switching days. She will work on Monday all day and I will work on Friday all day. Pam

Verified Credentials hours

Since things aren't moving the direction I want them, I have decided to change my hours there. I will no longer be blocked out on Wed mornings - I'll be here available to see patients starting this week. Additionally, on Monday's I will be open to see patients from 2:00 to 3:20, and I will go to VC after that for the end of the day. I will keep my Friday the same. I'll make the changes in the computer. Thanks for your flexibility.

Gift Cards w/ Ins.Funds

I called the help desk today and was told that the problem with the terminal not printing an insufficient funds receipt SHOULD be fixed. If anyone runs into that problem again please call the help desk while you have the card so you can "test" what they do and make sure it worked. Thank You...April

Jim's surgery/Janelle out

Hey all..

Here are the dates I know for sure that I will be out for Jim's pre-surgery appts and surgery:
3-17-08 out (pre-op and surgeon)
3-20-08 out by 11am(pre-surgery mri)
3-21-08 to3-28-08 out for surgery and hospital stay

I will let you know as I know for anything after 3-28-08

Employee MT cost

If you would like to "upgrade" your monthly 30 minute massage to an hour it will cost $15.

We are leaving town for Katrina's spring break. the last week of March. I will need fill in for Monday and Tuesday. Janelle is staying for the whole day on Monday fior me. April is trying to see if she can work Tues. am as well as the normal afternoon. If anyone can help fill in that would be great.
Also, I will be out Monday April 7. I can switch days with anyone else that week.

Medicare Replacement Policies

Here are the most common plans that have replaced Medicare policies:

Health Partners Freedom- these plans DO need to be prior auth.
Medica-Prime Solutions -PA as req by Medica
U-Care- Medicare Advantage
Blue Cross Blue Shield-Medicare Advantage

All these plans require medicare diagnosis and follow medicare guidelines (only adjustments covered/ no exams, therapies or x-rays) . These patients should still sign the ABN for these services.

Also, regarding the new fee slips: please make sure
Patient name is ledgible and that a date is being put on the top- if not by the patient, then by whoever is checking them in- this is our only signed record by the patient that they were in the office that day.

IF THEA KELLY CALLS FOR A MASSAGE APPT, PLEASE LET HER KNOW WE HAVE A 24 HR CANCELLATION POLICY. THIS WOULD BE THE 4TH APPT SHE WOULD MAKE WITHOUT SHOWING, SO WE NEED PAYMENT IN ADVANCE IN ORDER TO SCHEDULE AN APPT FOR HER.

Room Preference

For future reference - I would like to have all of my NP's start in room 1, even if I'm the only one in the office (Tues/Sat/etc). If that is not possible, I'll take what I can get, but that way I have my posters and personal info available for me and the patients to see. Thanks.

Collecting Sales Tax

Per our staff meeting on 1/14/08 regarding sales tax: According to the Minnesota Dept. of Revenue, we should be collecting sales tax of 6.5% on everything we sell here EXCEPT:
*Ace bandages
*Arch supports/insoles
*Braces of any kind
*Ice packs/ice bags
*Hot packs, heating pads

ABN/Waiver

Hi all -

Every Medicare patient is required (by Federal Law) to sign an ABN (Advanced Beneficiary Notice). This form states that some services are not covered by Medicare and the patient has the option to receive those services or not (and be required to pay for said service). They need to read and sign this form. Since we have not been doing this, please do your best to have each Medicare patient sign one on their next visit. After that, we need to have each Medicare patient sign one at least every two weeks. If a patient's visits are more than two weeks apart, they will need to sign one EVERY VISIT.

Also, we will be implementing a waiver that is mostly geared to HSM and Blue Plus patients, due to their reqirement stating patients must be informed that if their insurance does not cover certain services, they (the patient) will be financially responsible. If a patient is not made aware of this prior to the service, they (the patient) can not be billed and we are forced to 'eat' that bill and adjust it off their account. I realize this may be a little confusing, but these are not our rules, we simply need to follow them.

supplements

When entering supplement charges in Medisoft, please use the VIT code. All supplements will be charged sales tax, regardless of how the amount written on the bottle/box was determined. Supplements will continue to be 15% off, and I will make sure that the info about "how to get 20% off" is available by the end of the week. You can use the SUPPLY code for any supports/supplies, which will also be taxed automatically. All you will need to do is enter the amount of the charge (after taking appropriate discount), tax will automatically be entered...

New Charges

There are many new charges for the new year. Pay attention to the charge slip. (Example: Heat/Ice is now $20 not $16, Ifl is now $25 not $20).
Also, as of the first of the year Medicare patients will be responsible for the full amount of the
x-ray charge ($40/ xray)
